Inc. has a fast and easy process.
Begin by
deciding on the service plan that suits you best: Insurance
Only or Comprehensive Billing
Upon request
we will send you a complete registration package. This includes the New
Provider Form (for your professional demographic information), our
full pricing schedule, our Service Agreement and blank copies of the New
Patient Demographic Form as well as the easy Claim
and Payment Form. Please fill out and return the New
Provider Form and the Service Agreement along with complete demographic
information for all your active patients. This can generally be accomplished
through fax or mail.
Now all that
remains is this simple, weekly process:

Reporting billable sessions

Fax this using our quick and easy Claim
and Payment form. |

2)
Reporting insurance payments you receive
|
| |
|
Fax us the EOB (Evidence of Billing) documentation you receive from
insurance companies. The EOB lists all sessions claimed and amounts
paid. It will be attached to the checks you receive. We will cross-check
this information with claims we submitted on your behalf to be sure
each claim was paid in full. |

3)
Notifying us of new patients as they enter your practice

There
are two ways to do this. Ask new patients to call us directly or
fill out and fax our New
Patient form.
a)
Insurance-Only plan: Submit forms only for your patients who use
insurance.
b)
Comprehensive Billing plan: Submit forms for ALL new patients. Include
any private-pay fees you have set.

For the Comprehensive
Billing plan there is one last step:

Notifying us of out-of-pocket payments (both co-pays and fees) you
receive
|
| |
|
Because you will use the same Claim
and Payment form you use to report your billable sessions, this
step could not be simpler. This will let us track patient balances. |
For either
of our two service plans nothing more is required.
